12/12/2023 EnchantaVaughn Approved QMHPA arrived to the home of Chevelle. Upon arrival to the home, Chevelle appeared to be in a relaxed mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. She verbalized to QMHPA that her morning was going well. Chevelle did not present any symptoms of SI or HI. QMHPA began the session by inquiring about her current thoughts, feelings and emotions. QMHPA processed Chevelle feelings and emotions with improving her overall daily functioning and stabilize her mental and medical health management. QMHPA praised Chevelle for her improvement of being able to communicate with QMHPA about her thoughts and feelings and any learned coping skills as she continues to learn to assertively express her unmet emotional needs and stressors. QMHPA reiterated the importance of Chevelle being able to recognize and identify stressors that cause her inability to stabilize her mental and medical health management on a daily basis. QMHPA assisted Chevelle with reaching a level of reduced tension to increase and improve communication with family and friends for improvement in overall daily functioning. QMHPA assisted Chevelle with learning to recognize and verbalize hurt and angry feelings in open, respectful constructive ways. Before ending the session, QMHPA assisted Chevelle in becoming able to recognize needs and feels and to express them in a healthy way. Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Chevelle stated to QMHPA that she does comply with her medication but as she mentioned to QMHPA at the initial session, she does not adhere to her medication on a consistent basis because she doesn’t like the way that it makes her feel. Chevelle was able to provide QMHPA with learned coping skills when her needs are unmet as well as when she has stressors that she has to deal with; however, she indicated that she doesn’t always use those coping skills because she is quick to go off on the edge. Chevelle stated to QMHPA that doesn’t like communicating with her family members much because they are always bumping heads so she tries to avoid those “dysfunctional folk”. At the end of the session, Chevelle was able to verbalize her feeling and recognize her needs; however, she needed several prompts and assistance from QMHPA.
